The Power of Baroque Music as Help for Anxiety

Baroque music i.e. music composed during the Baroque era (1600 - 1750) has been shown to have a calming effect on the brain. It is generally the slow movements of such music which has this effect.

It is believed to be it does this because the rhythm is around 60 beats per minute which is similar to a resting heartrate. The brain 'entrains' itself to rhythm. We have all noticed the tendency of our foot to tap when we hear certain types of music. This is the brain buying into the rhythm and going along with it.


We all have brainwaves which move at different frequencies depending what we are doing or thinking. An EEG recording is similar to the more familiar ECG for measuring the heart. With an EEG, however, the electrodes are placed on the head.

This is a painless procedure. The electrodes gives a readout of wiggly lines. These show the frequency of the brainwaves produced in the different areas.

What do the different brainwave frequencies mean?


Brainwaves in a human vary from beta waves (which are upwards of 15 cycles per second) and appear very spiky, through alpha waves, theta waves, and delta waves. 

Most of the time we are awake and participating in everyday life we have beta waves.
When we are deeply asleep we have a high percentage of delta waves which are like a gently rolling ocean.

So as we fall asleep our brainwaves gradually slow through all the stages to delta. Then they move back up again as we wake.

When we dream we are usually in theta and then if this is just one dream period we then go deeper again. There is a diagram of this on the page that explains nightmares.
Alpha waves are the daytime magic ones, when we are awake.

It is during alpha that we can access our unconscious thoughts. During hypnosis the client goes into the alpha state so that their issues may be accessed without the thinking and rationalising part of the brain getting in the way.

It is thought that it is during our dreams when we sleep that we resolve issues of the day and that is done during theta.

How do we know all this?


Research in Bulgaria have found that slow Baroque music entrains the brain into alpha waves. Studies have been done whereby such music, when played in the background during lessons in junior school classrooms, has a calming effect and enables children to concentrate on their lesson far better.

I have sometimes given patients suffering from PTSD or other severe anxieties which stopped them sleeping, or gave them constant nightmares, a CD of Baroque music with the instruction to just play it quietly in the background when they go to bed. In many cases this has had a significant effect and has enabled them to sleep better, often through the night, and to reduce the occurrence of nightmares.

To feel the calming effect of this music you don't have to consciously listen to it but to allow it to play softly in the background as you do everyday tasks. 

I have found some commercially available cds of this music which are available to buy should you wish. The most famous Baroque (1600-1760) composers are probably Bach, Vivaldi, Pachelbel, Handel Telemann, Purcell, Scarlatti. It is the slow, largo, movements of their works which are relevant here. There are few CDs available at present which are made up exclusively of such tracks so you may have to put together your own. Some of the tracks can be purchased individually as mp3 downloads on Amazon and similar sites.

If you feel inclined to listen to largo music combined with the sounds of nature, then I can recommend the Solitudes series by Dan Gibson, which do just that. There are two CDs of music by Pachelbel, Forever by the Sea and In the Garden as well as Bach Forever by the Sea which incorporates his music with the sounds of the ocean. Can cognitive behavioural therapy really change our brains? (BBC)


Cognitive behavioural therapy (CBT) is a type of talking therapy that's used to treat a wide range of mental health problems, from depression and eating disorders to phobias and obsessive-compulsive disorder (OCD). It recommends looking at ourselves in a different way that might prove useful for all of us in everyday life. But what happens to our brains when we have CBT?

What is cognitive behavioural therapy?

CBT is based on the idea that problems aren't caused by situations themselves, but by how we interpret them in our thoughts. These can then affect our feelings and actions.

For example, if someone you know walks by without saying hello, what's your reaction?

You might think that they ignored you because they don't like you, which might make you feel rejected. So you might be tempted to avoid them the next time you meet. This could breed more bad feeling between you both and more "rejections", until eventually you believe that you must be unlikeable. If this happened with enough people, you could start to withdraw socially.

But how well did you interpret the situation in the first place?
 

Common errors in thinking style

  • Emotional reasoning - e.g. I feel guilty so I must be guilty
  • Jumping to conclusions - e.g. if I go into work when I'm feeling low, I'll only feel worse
  • All-or-nothing thinking - e.g. if I've not done it perfectly, then it's absolutely useless
  • Mental filtering - e.g. noticing my failures more than my successes
  • Over generalising - e.g. nothing ever goes well in my life
  • Labelling - e.g. I'm a loser
CBT aims to break negative vicious cycles by identifying unhelpful ways of reacting that creep into our thinking. 

"Emotional reasoning is a very common error in people's thinking," explains Dr Jennifer Wild, Consultant Clinical Psychologist from Kings College London. "That's when you think something must be true because of how you feel."

CBT tries to replace these negative thinking styles with more useful or realistic ones.
This can be a challenge for people with mental health disorders, as their thinking styles can be well-established.

How do we break negative thinking styles?

Some psychological theories suggest that we learn these negative thinking patterns through a process called negative reinforcement.

For example, if you have a fear of spiders, by avoiding them you learn that your anxiety levels can be reduced. So you're rewarded in the short term with less anxiety but this reinforces the fear.

To unlearn these patterns, people with phobias and anxiety disorders often use a CBT technique called graded exposure. By gradually confronting what frightens them and observing that nothing bad actually happens, it's possible to slowly retrain their brains to not fear it.

How does cognitive behavioural therapy work on the brain?

Primitive survival instincts like fear are processed in a part of the brain called the limbic system. This includes the amygdala, a region that processes emotion, and the hippocampus, a region involved in reliving traumatic memories.

Brain scan studies have shown that overactivity in these two regions returns to normal after a course of CBT in people with phobias. 

What's more, studies have found that CBT can also change the prefrontal cortex, the part of the brain responsible for higher-level thinking.

So it seems that CBT might be able to make real, physical changes to both our "emotional brain" (instincts) and our "logical brain" (thoughts). 

Intriguingly, similar patterns of brain changes have been seen with CBT and with drug treatments, suggesting that psychotherapies and medications might work on the brain in parallel ways.

How effective is cognitive behavioural therapy? 

Of all the talking therapies, CBT has the most clinical evidence to show that it works. 

Studies have shown that it is at least as effective as medication for many types of depression and anxiety disorders. 

But unlike many drugs, there are few side effects with CBT. After a relatively short course, people have often described long-lasting benefits. 

"In the trials we've run with post-traumatic stress disorder [PTSD] and social anxiety disorder, we've seen that even when people stop the therapy, they continue improving because they have new tools in place and they've made behavioural and thinking style changes," Dr Wild explains.

CBT may not be for everyone, however. 

Since the focus is on tackling the here and now, people with more complicated roots to their mental problems which could stem from their childhood, for example, may need another type of longer-term therapy to explore this. 

CBT also relies on commitment from the individual, including "homework" between therapy sessions. It can also involve confronting fears and anxieties, and this isn't always easy to do. 

Ultimately, as with many types of treatment, some people will benefit from CBT more than others and psychologists and neuroscientists are beginning to unravel the reasons behind this.

Health benefits of music (net doctor)


Music has a special power to move us and stir our emotions. Anyone who has ever wiped tears away from their eyes listening to their favourite sad song will know how powerful simple notes and chords can be. 

Now, scientific studies have shown that music really can change our mood and even help us concentrate.

We look at the effects music can have, and we ask the experts what songs are likely to help you run a race, prepare for an exam or relieve stress.

Music matters

Listening to a song can have a real effect on various parts of the brain, with studies showing that areas responsible for aspects, such as memory and vision, can 'light up' in response to music. 

'There's a very wide range of reactions in the body and mind to music, and brain imaging studies have shown that various parts of the brain may be activated by a piece of music,' says Dr Victoria Williamson, lecturer in psychology at Goldsmith's College, London.

'For example, a recent study in Canada showed that there's a real causal relationship between music and the reward system, a core part of the brain that reacts to stimuli, which are good for us – food, light, sex for example – and reinforces these behaviours meaning that we do them more.'

So what benefits can music bring?

Some people listen to music for a boost on a tough day, while others might use music to keep them awake during a long car journey.  

Everyone reacts to music in different ways. One individual may love heavy metal for example, while another is happiest listening to Mozart. 

Whatever your preference, a 2011 Canadian study, published in Nature Neuroscience, has shown that plugging in to your favourite music could help melt away a bad mood. 

Researchers at McGill University in Montreal showed that listening to pleasurable music of any description induced 'musical chills', which triggered the release of the feel-good chemical dopamine. 

'We all know from our own individual experiences that listening to music can affect mood,' says Bridget O'Connell, head of information at the mental health charity Mind. 

'Some people listen to music for a boost on a tough day, while others might use music to keep them awake during a long car journey or to purge a negative feeling. 

'The brain is very complicated – and there are many elements involved in feelings of pleasure – but it's unsurprising that research suggests dopamine release is linked with feelings of pleasure induced by music.'

Focus

Music may even be able to help you concentrate. 

A new 'digital tonic' called Ubrain, which can be downloaded onto smartphones, claims to be able to help people focus, energise, wake up as well as relax. 

The process uses two different beats in each ear to create a third 'perceived' beat (a binaural beat), which can stimulate certain activity in the brain. 

'By helping the brain cortex to generate specific brain waves, we can induce different states of alertness, depending on what we aim to do,' explains Paris-based clinical psychologist Brigitte Forgeot. 

'If we're feeling anxious or stressed, we can encourage our cerebral cortex to produce slow alpha-frequency brain waves, while on the other end of the scale, if we help our cortex to produce faster beta waves, we will be better equipped to concentrate and focus our attention on a fairly lengthy task.'

A study at Brunel University in West London has shown that music can help increase endurance by as much as 15 per cent, helping to lower the perception of effort during exercise, as well as increasing energy efficiency by between one and three per cent.

The best choices for exercise are up-beat songs that match the tempo of your running stride and which can have a metronomic effect on the body, enabling you to run for longer.

Better mental health

Music can be an effective and positive treatment for people dealing with mental health conditions. 

'There are two distinct ways music therapy is used: either as a means of communication and self-expression or for its inherent restorative or healing qualities,' says Bridget O'Connell.

'Someone who is very withdrawn may find that music can act as an outlet for expressing things that they're unable to put into words. It can also act as a stimulus to awaken buried memories or evoke emotional responses that may take weeks to achieve with talking therapies.'

According to 2011 figures from the mental health charity Mind, nearly a third of people plugged into their music players to give them a mood boost about work, and almost one in four said that they find listening to music on the way to the workplace helps them de-stress.

Paul Farmer, the charity's CEO, backs up the statistics by saying that the therapeutic benefits of listening to music are well-known.

Tuning in to one of your favourite songs can be incredibly soothing and help to reduce anxiety.

Patient care

Music can actually have a significant positive impact on patients with long-term illnesses, such as heart disease, cancer and respiratory conditions

Numerous trials have shown that music can help lower heart rate, blood pressure and help relieve pain, anxiety and improve patient quality of life. 

'Music can be incredibly useful for somebody who is in a situation where they have lost a lot of control from their external environment – say they are in hospital for a long period of time with a serious illness and less able to move around,' says Dr Williamson.

'It can give them a sense of control back, as well as creating a calm personal atmosphere and blocking out some of the disturbances around the patient.'

Which music?

While there are certain trends – fast upbeat music for exercising and slower-paced music to relax – choosing songs that have the desired effect is often linked to personal preference.

'The effect of different types of music on mood will largely depend on people's individual preference and experience,' says Bridget O'Connell. 

'If you grow up with rock music, you might not find classical music uplifting at all. On the flipside, some people can't bear rock music, so they are more likely to be wound up than uplifted.

'Music can also invoke particular memories for people, including some that could potentially make them upset. On the other hand, it could also bring them out of a severely withdrawn state or act as a form of communication in place of words.'

There are some rules of thumb though, admits Dr Williamson. 'For a general rule, if you want to relax you should choose songs with slower tempo, less key changes and more predictable structure.'

Alzheimer’s & dementia prevention pillar #4: Quality sleep


Your brain needs regular, restful sleep in order to function at optimum capacity. Sleep deprivation not only leaves you cranky and tired, but impairs your ability to think, problem-solve, and process, store, and recall information. Deep, dreamy sleep is critical for memory formation and retention. If nightly sleep deprivation is slowing your thinking and affecting your mood, you may be at greater risk of developing symptoms of Alzheimer’s disease. The vast majority of adults need at least 8 hours of sleep per night. Any less, and productivity and creativity suffers.

Tips to help you combat insomnia and catch up on your Z’s

  • Establish a regular sleep schedule. Going to bed and getting up at the same time reinforces your natural circadian rhythms. Your brain’s clock responds to regularity.
  • Be smart about napping. While taking a nap can be a great way to recharge, especially for older adults, it can make insomnia worse. If insomnia is a problem for you, consider eliminating napping. If you must nap, do it in the early afternoon, and limit it to thirty minutes.
  • Set the mood. Reserve your bed for sleep and sex, and ban television and computers from the bedroom (both are stimulating and may lead to difficulties falling asleep).
  • Create a relaxing bedtime ritual. Take a hot bath, do some light stretches, write in your journal, or dim the lights. As it becomes habit, your nightly ritual will send a powerful signal to your brain that it’s time for deep restorative sleep.
  • Quiet your inner chatter. When stress, anxiety, or negative internal dialogues keep you awake, get out of bed. Try reading or relaxing in another room for twenty minutes then hop back in.

Relaxation technique 6: Yoga and tai chi for stress relief


Yoga involves a series of both moving and stationary poses, combined with deep breathing. As well as reducing anxiety and stress, yoga can also improve flexibility, strength, balance, and stamina. Practiced regularly, it can also strengthen the relaxation response in your daily life. Since injuries can happen when yoga is practiced incorrectly, it’s best to learn by attending group classes, hiring a private teacher, or at least following video instructions.

What type of yoga is best for stress?

Although almost all yoga classes end in a relaxation pose, classes that emphasize slow, steady movement, deep breathing, and gentle stretching are best for stress relief.
  • Satyananda is a traditional form of yoga. It features gentle poses, deep relaxation, and meditation, making it suitable for beginners as well as anyone primarily looking for stress reduction.
  • Hatha yoga is also reasonably gentle way to relieve stress and is suitable for beginners. Alternately, look for labels like gentle, for stress relief, or for beginners when selecting a yoga class.
  • Power yoga, with its intense poses and focus on fitness, is better suited to those looking for stimulation as well as relaxation.

Tai chi

If you’ve ever seen a group of people in the park slowly moving in synch, you’ve probably witnessed tai chi. Tai chi is a self-paced, non-competitive series of slow, flowing body movements. These movements emphasize concentration, relaxation, and the conscious circulation of vital energy throughout the body. Though tai chi has its roots in martial arts, today it is primarily practiced as a way of calming the mind, conditioning the body, and reducing stress. As in meditation, tai chi practitioners focus on their breathing and keeping their attention in the present moment.

Tai chi is a safe, low-impact option for people of all ages and levels of fitness, including older adults and those recovering from injuries. Like yoga, once you’ve learned the basics of tai chi or qi gong, you can practice alone or with others, tailoring your sessions as you see fit. 

Relaxation technique 5: Visualization meditation for stress relief


Visualization, or guided imagery, is a variation on traditional meditation that requires you to employ not only your visual sense, but also your sense of taste, touch, smell, and sound. When used as a relaxation technique, visualization involves imagining a scene in which you feel at peace, free to let go of all tension and anxiety.

Choose whatever setting is most calming to you, whether it’s a tropical beach, a favorite childhood spot, or a quiet wooded glen. You can do this visualization exercise on your own in silence, while listening to soothing music, or with a therapist (or an audio recording of a therapist) guiding you through the imagery. To help you employ your sense of hearing you can use a sound machine or download sounds that match your chosen setting—the sound of ocean waves if you’ve chosen a beach, for example.

Practicing visualization

Find a quiet, relaxed place. Beginners sometimes fall asleep during a visualization meditation, so you might try sitting up or standing.

Close your eyes and let your worries drift away. Imagine your restful place. Picture it as vividly as you can—everything you can see, hear, smell, and feel. Visualization works best if you incorporate as many sensory details as possible, using at least three of your senses. When visualizing, choose imagery that appeals to you; don’t select images because someone else suggests them, or because you think they should be appealing. Let your own images come up and work for you.

If you are thinking about a dock on a quiet lake, for example:
  • Walk slowly around the dock and notice the colors and textures around you.
  • Spend some time exploring each of your senses.
  • See the sun setting over the water.
  • Hear the birds singing.
  • Smell the pine trees.
  • Feel the cool water on your bare feet.
  • Taste the fresh, clean air.
Enjoy the feeling of deep relaxation that envelopes you as you slowly explore your restful place. When you are ready, gently open your eyes and come back to the present.

Don't worry if you sometimes zone out or lose track of where you are during a guided imagery session.  This is normal. You may also experience feelings of stiffness or heaviness in your limbs, minor, involuntary muscle-movements, or even cough or yawn. Again, these are normal responses. 

Relaxation technique 1: Breathing meditation for stress relief


With its focus on full, cleansing breaths, deep breathing is a simple, yet powerful, relaxation technique. It’s easy to learn, can be practiced almost anywhere, and provides a quick way to get your stress levels in check. Deep breathing is the cornerstone of many other relaxation practices, too, and can be combined with other relaxing elements such as aromatherapy and music. All you really need is a few minutes and a place to stretch out.

Practicing deep breathing meditation

The key to deep breathing is to breathe deeply from the abdomen, getting as much fresh air as possible in your lungs. When you take deep breaths from the abdomen, rather than shallow breaths from your upper chest, you inhale more oxygen. The more oxygen you get, the less tense, short of breath, and anxious you feel.
  • Sit comfortably with your back straight. Put one hand on your chest and the other on your stomach.
  • Breathe in through your nose. The hand on your stomach should rise. The hand on your chest should move very little.
  • Exhale through your mouth, pushing out as much air as you can while contracting your abdominal muscles. The hand on your stomach should move in as you exhale, but your other hand should move very little.
  • Continue to breathe in through your nose and out through your mouth. Try to inhale enough so that your lower abdomen rises and falls. Count slowly as you exhale.
If you find it difficult breathing from your abdomen while sitting up, try lying on the floor. Put a small book on your stomach, and try to breathe so that the book rises as you inhale and falls as you exhale. 


Meet Your Brain Waves — Introducing Alpha, Beta, Theta, Delta, And Gamma


It’s important to understand how your brain contributes to the state of your mind. While most of us focus on looking at our emotions in an attempt to become happier, more spiritual beings, our brains waves and our subconscious mind also play a key part in our quest for fulfillment. 

In this article, we’ll be looking at our five brain wave frequencies and how they affect our state of mind, and will be following up on Thursday with a more in-depth look at the impact they have on us physically and mentally, in addition to some exercises we can do to “switch on” certain frequencies.
Are We The Controllers Of Our Reality?
We easily forget that we are the controllers of our reality – and that “our reality” is not made up of outside influences, but that it actually consists of our thoughts, beliefs and mindset.

Therefore, by learning about the deeper states of consciousness, you can open your subconscious mind and create your reality at will, and with precision. To do this, the first step is understanding your different brain frequencies. Did you know that we all have five (Beta, Alpha, Theta, Delta and Gamma), and each frequency is measured in cycles per second (Hz) and has its own set of characteristics representing a specific level of brain activity and a unique state of consciousness? Smart, eh?
1) Beta (14-40Hz) – The Waking Consciousness And Reasoning Wave
 
Beta brain waves are associated with normal waking consciousness and a heightened state of alertness, logic and critical reasoning.

While Beta brain waves are important for effective functioning throughout the day, they also can translate into stress, anxiety and restlessness.

The voice of Beta can be described as being that nagging little inner critic that gets louder the higher you go into range. Therefore, with a majority of adults operate at Beta; it’s little surprise that stress is today’s most common health problem.
2) Alpha (7.5-14Hz) – The Deep Relaxation Wave

Alpha brain waves are present in deep relaxation and usually when the eyes are closed, when you’re slipping into a lovely daydream or during light meditation. It is an optimal time to program the mind for success and it also heightens your imagination, visualization, memory, learning and concentration.

It is the gateway to your subconscious mind and lies at the base of your conscious awareness. The voice of Alpha is your intuition, which becomes clearer and more profound the closer you get to 7.5Hz.

The renowned Silva Method, by Jose Silva is premised on the power of Alpha. However the Silva Method allows you to achieve this deep relaxation through meditation during waking consciousness.

3) Theta (4-7.5Hz) – The Light Meditation And Sleeping Wave
Theta brain waves are present during deep meditation and light sleep, including the all-important REM dream state. It is the realm of your subconsciousness and only experienced momentarily as you drift off to sleep from Alpha and wake from deep sleep (from Delta).

It is said that a sense of deep spiritual connection and unity with the universe can be experienced at Theta. Your mind’s most deep-seated programs are at Theta and it is where you experience vivid visualizations, great inspiration, profound creativity and exceptional insight. Unlike your other brain waves, the elusive voice of Theta is a silent voice.

It is at the Alpha-Theta border, from 7Hz to 8Hz, where the optimal range for visualization, mind programming and using the creative power of your mind begins. It’s the mental state which you consciously create your reality. At this frequency, you are conscious of your surroundings however your body is in deep relaxation.
4) Delta (0.5-4Hz) – The Deep Sleep Wave
 
The Delta frequency is the slowest of the frequencies and is experienced in deep, dreamless sleep and in very deep, transcendental meditation where awareness is fully detached.

Delta is the realm of your unconscious mind, and the gateway to the universal mind and the collective unconscious, where information received is otherwise unavailable at the conscious level.

Among many things, deep sleep is important for the healing process – as it’s linked with deep healing and regeneration. Hence, not having enough deep sleep is detrimental to your health in more ways than one.
5) Gamma (above 40Hz) – The Insight Wave
This range is the most recently discovered and is the fastest frequency at above 40Hz. While little is known about this state of mind, initial research shows Gamma waves are associated with bursts of insight and high-level information processing.

What is a phobia? (BBC)


A phobia is an intense fear or feeling of anxiety that occurs only in a particular situation that frightens you. 

This might be something as seemingly logical as a fear of heights, or as illogical as a fear of the colour green. At other times you don't feel anxious. For example, if you have a phobia of spiders (as millions of people do), you only feel anxious when there's a spider around, otherwise you feel fine. 

About one in ten people has a significant phobia, although few people seek treatment.

People develop phobias to all sorts of things. Each phobia has its own name. Some (of a very long list) include:
  • Musophobia - fear of mice
  • Peladophobia - fear of bald people
  • Amathophobia - fear of dust
  • Pnigophobia - fear of choking or smothering
  • Maieusiophobia - fear of childbirth
  • Homichlophobia - fear of fog
  • Arachibutyrophobia - fear of peanut butter sticking to the roof of the mouth
Phobias make people avoid situations they know will make them anxious, but this can make the phobia worse. A person's life can become increasingly dominated by the precautions they take to avoid a situation they fear. You may know there's no real danger and you may feel embarrassed by your fear, but you're still unable to control it. It's better to confront your fears, even if it's in a very careful way or with the help of a trained therapist. 

A phobia is more likely to go away if it began after a distressing or traumatic event.

What's the treatment?

Cognitive behavioural therapy - a 'talking treatment' where you learn all about the thing or situation you are scared of and how to change your behaviour - has a high success rate in phobias. Your GP can refer you.

Antidepressant medications may also be used even if you aren't depressed. They work by interfering with brain chemicals (neurotransmitters) such as serotonin which may be involved in causing anxiety symptoms.

Anti-anxiety medication such as diazepam are rarely used, as they can be addictive, but may be offerred on ‘one-off’ occasions such as for people with flying phobias who have to take a flight for work or holiday.

Obsessive-Compulsive Disorder (kidshealth.org)


All kids have worries and doubts. But kids with obsessive-compulsive disorder (OCD) often can't stop worrying, no matter how much they want to. And those worries frequently compel them to behave in certain ways over and over again.

About OCD

OCD is a type of anxiety disorder. Kids with OCD become preoccupied with whether something could be harmful, dangerous, wrong, or dirty — or with thoughts that bad stuff could happen.

With OCD, upsetting or scary thoughts or images, called obsessions, pop into a person's mind and are hard to shake. Kids with OCD also might worry about things not being "in order" or "just right." They may worry about losing things, sometimes feeling the need to collect these items, even though they may seem useless to other people.

Someone with OCD feels strong urges to do certain things repeatedly — called rituals or compulsions — in order to banish the scary thoughts, ward off something dreaded, or make extra sure that things are safe, clean, or right in some way.

Children may have a difficult time explaining a reason for their rituals and say they do them "just because." But in general, by doing a ritual, someone with OCD is trying to relieve anxiety. They may want to feel absolutely certain that something bad won't happen or to feel "just right."

Think of OCD as an "overactive alarm system." The rise in anxiety or worry is so strong that a child feels like he or she must perform the task or dwell on the thought, over and over again, to the point where it interferes with everyday life.

Most kids with OCD realize that they really don't have to repeat the behaviors over and over again, but the anxiety can be so great that they feel that repetition is "required" to neutralize the uncomfortable feeling. And often the behavior does decrease the anxiety — but only temporarily. In the long run, the rituals may worsen OCD severity and prompt the obsessions to return.

Causes

Doctors and scientists don't know exactly what causes OCD, although recent research has led to a better understanding of it and its potential causes. Experts believe OCD is related to levels of a neurotransmitter called serotonin. Neurotransmitters are chemicals that carry signals in the brain.

When the flow of serotonin is blocked, the brain's "alarm system" overreacts and misinterprets information. These "false alarms" mistakenly trigger danger messages. Instead of the brain filtering out these messages, the mind dwells on them — and the person experiences unrealistic fear and doubt.

Evidence is strong that OCD tends to run in families. Many people with OCD have one or more family members who also have it or other anxiety disorders influenced by the brain's serotonin levels. Because of this, scientists have come to believe that the tendency (or predisposition) for someone to develop a serotonin imbalance that causes OCD can be inherited.

Having the genetic tendency for OCD doesn't mean that someone will develop OCD, but it does mean there's a stronger chance that he or she might. Sometimes an illness or some other stress-causing event may trigger the symptoms of OCD in a person who is genetically prone to develop it.

It's important to understand that the obsessive-compulsive behavior is not something that a child can stop by trying harder. OCD is a disorder, just like any physical disorder such as diabetes or asthma, and is not something kids can control or have caused themselves.

OCD is also not something that parents have caused, although life events (such as starting school or the death of a loved one) might worsen or trigger the onset of OCD in kids who are prone to develop it.

Common OCD Behaviors in Kids

OCD can make daily life difficult for the kids that it affects and their families. The behaviors often take up a great deal of time and energy, making it more difficult to complete tasks, such as homework or chores, or to enjoy life.

In addition to feeling frustrated or guilty for not being able to control their own thoughts or actions, kids with OCD also may suffer from low self-esteem or from shame or embarrassment about what they're thinking or feeling (since they often realize that their fears are unrealistic, or that their rituals are not realistically going to prevent their feared events).

They also may feel pressured because they don't have enough time to do everything. A child might become irritable because he or she feels compelled to stay awake late into the night or miss an activity or outing to complete the compulsive rituals. Kids might have difficulties with attention or concentration because of the intrusive thoughts.

Among kids and teens with OCD, the most common obsessions include:
  • fear of dirt or germs
  • fear of contamination
  • a need for symmetry, order, and precision
  • religious obsessions
  • preoccupation with body wastes
  • lucky and unlucky numbers
  • sexual or aggressive thoughts
  • fear of illness or harm coming to oneself or relatives
  • preoccupation with household items
  • intrusive sounds or words
These compulsions are the most common among kids and teens:
  • grooming rituals, including hand washing, showering, and teeth brushing
  • repeating rituals, including going in and out of doorways, needing to move through spaces in a special way, or rereading, erasing, and rewriting
  • checking rituals to make sure that an appliance is off or a door is locked, and repeatedly checking homework
  • rituals to undo contact with a "contaminated" person or object
  • touching rituals
  • rituals to prevent harming self or others
  • ordering or arranging objects
  • counting rituals
  • hoarding and collecting things of no apparent value
  • cleaning rituals related to the house or other items

Signs and Symptoms of OCD

Recognizing OCD is often difficult because kids can become adept at hiding the behaviors. It's not uncommon for a child to engage in ritualistic behavior for months, or even years, before parents know about it. Also, a child may not engage in the ritual at school, so parents might think it's just a phase.

When a child with OCD tries to contain these thoughts or behaviors, this creates anxiety. Kids who feel embarrassed or as if they're "going crazy" may try to blend the OCD into the normal daily routine until they can't control it anymore.

It's common for kids to ask a parent to join in the ritualistic behavior: First the child has to do something and then the parent has to do something else. If a child says, "I didn't touch something with germs, did I?" the parent might have to respond, "No, you're OK," and the ritual will begin again for a certain number of times. Initially, the parent might not notice what is happening.

Tantrums, overt signs of worry, and difficult behaviors are common when parents fail to participate in their child's rituals. It is often this behavior, as much as the OCD itself, which brings families into treatment.

Parents can look for the following possible signs of OCD:
  • raw, chapped hands from constant washing
  • unusually high rate of soap or paper towel usage
  • high, unexplained utility bills
  • a sudden drop in test grades
  • unproductive hours spent doing homework
  • holes erased through test papers and homework
  • requests for family members to repeat strange phrases or keep answering the same question
  • a persistent fear of illness
  • a dramatic increase in laundry
  • an exceptionally long amount of time spent getting ready for bed
  • a continual fear that something terrible will happen to someone
  • constant checks of the health of family members
  • reluctance to leave the house at the same time as other family members

Diagnosing OCD

OCD is more common than many other childhood disorders or illnesses, but it often remains undiagnosed. Kids might keep the symptoms hidden from their families, friends, and teachers because they're embarrassed.

Even when symptoms are present, a parent or health care provider might not recognize that they are part of a mental health disorder and may attribute them to a child's quirkiness or even bad behavior.

Doctors consider OCD to be a pattern of obsessive thinking and rituals that does one or more of the following:
  • takes up more than an hour each day
  • causes distress
  • interferes with daily activities
OCD in kids is usually diagnosed between the ages of 7 and 12. Since these are the years when kids naturally feel concerned about fitting in with their friends, the discomfort and stress brought on by OCD can make them feel scared, out of control, and alone.

If your child shows signs of OCD, talk to your doctor. In screening for OCD, the doctor or a mental health professional will ask your child about obsessions and compulsions in language that kids will understand, such as:
  • Do you have worries, thoughts, images, feelings, or ideas that bother you?
  • Do you have to check things over and over again?
  • Do you have to wash your hands a lot, more than most kids?
  • Do you count to a certain number or do things a certain number of times?
  • Do you collect things that others might throw away (like hair or fingernail clippings)?
  • Do things have to be "just so"?
  • Are there things you have to do before you go to bed?
Because it might be normal for a child who doesn't have OCD to answer yes to any of these questions, the doctor also will ask about how often and how severe the behaviors are, about your family's history of OCD, Tourette syndrome and other motor or vocal tic disorders, or other problems that sometimes occur with OCD. OCD is common in people with Tourette syndrome.

Other disorders that often occur with OCD include other anxiety disorders, depression, disruptive behavior disorders, attention deficit hyperactivity disorder (ADHD), learning disorders, and trichotillomania (compulsive hair pulling). PANS, a rare condition that stands for Pediatric Acute-onset Neuropsychiatric Syndrome, also has been associated with having OCD. 

Treating OCD

The most successful treatments for kids with OCD are behavioral therapy and medication. Behavioral therapy, also known as cognitive-behavioral psychotherapy (CBT), helps kids learn to change thoughts and feelings by first changing behavior.

Behavioral therapy involves gradually exposing kids to their fears, with the agreement that they will not perform rituals, to help them recognize that their anxiety will eventually decrease and that no disastrous outcome will occur. For example, kids who are afraid of dirt might be exposed to something dirty, starting with something mildly bothersome and ending with something that might be really dirty.

For exposure to be successful, it must be combined with response prevention, in which the child's rituals or avoidance behaviors are blocked. For example, a child who fears dirt must not only stay in contact with the dirty object, but also must not be allowed to wash repeatedly.

Some treatment plans involve having the child "bossing back" the OCD, giving it a nasty nickname, and visualizing it as something he or she can control. Over time, the anxiety provoked by dirt and the urge to perform washing rituals gradually disappear. The child also gains confidence that he or she can "fight" OCD.

OCD can sometimes worsen if it's not treated in a consistent, logical, and supportive manner. So it's important to find a therapist who has training and experience in treating OCD.

Just talking about the rituals and fears have not been shown to help OCD, and may actually make it worse by reinforcing the fears and prompting extra rituals. Family support and cooperation also go a long way toward helping a child cope with OCD.

Many kids can do well with behavioral therapy alone while others will need a combination of behavioral therapy and medication. Therapy can help your child and family learn strategies to manage the ebb and flow of OCD symptoms, while medication, such as selective serotonin reuptake inhibitors (SSRIs), often can reduce the impulse to perform rituals.

Helping Kids With OCD

It's important to understand that OCD is never a child's fault. Once a child is in treatment, it's important for parents to participate, to learn more about OCD, and to modify expectations and be supportive.

Kids with OCD get better at different rates, so try to avoid any day-to-day comparisons and recognize and praise any small improvements. Keep in mind that it's the OCD that is causing the problem, not the child. The more that personal criticism can be avoided, the better.

It can be helpful to keep family routines as normal as possible, and for all family members to learn strategies to help the child with OCD. It's also important to not let OCD be the "boss" of the house and regular family activities. Giving in to OCD worries does not make them go away.
